Payment

The deposit to reserve a new service appointment is $50, unless stated otherwise. Deposit payments are NON-REFUNDABLE and non-transferable to another person.

The deposit payment is deducted from your service total. The remaining balance will be collected on the day of your appointment, prior to your procedure. Payments can be made in Cash and/or Credit/Debit. Electronic money transferring services will not be accepted for any reason. Under no circumstance will this payment be considered for a refund. All payments are final.

The touch up fee ($100) is separate. This fee is NOT charged on your initial visit investment. Therefore, you must book and pay a separate deposit ($50) for the touchup.

Scheduling

In order to reschedule an appointment a 72 hours’ notice is required and enforced. If appointment is cancelled within this time frame, you may transfer your deposit to reserve a new date. If appointment is cancelled without properly rescheduling, the deposit is non-refundable and appointment will be lost. In addition, appointment availability for rescheduled appointments is not guaranteed. It is advised to speak to Azucena personally and in a timely manner should a scheduling conflict occur.

Tardiness is neither accepted nor tolerated. Any appointment running greater than 15 minutes late will automatically be cancelled and all money collected will be forfeited. Failure to comply will result in a missed appointment. Keep in mind the artist needs sufficient time for the procedure. Any reduction from the allotted time hinders the artist's ability to adequately complete the procedure and may deter ideal results.

Touch up appointments are recommended to achieve ideal results. This service is NOT automatically booked. This service must be self-booked by the client within 6-12 weeks from the original appointment date to avoid additional fees. We recommend booking this appointment when you book your initial appointment to avoid schedule conflicts.

Azucena Yearby, Dr. Martinez, Uptown Smiles, Mekiero, LLC nor any affiliates of Mekiero offers NO GUARANTEE for permanent makeup results.  

Results will vary from client to client on an individual basis.

Your skin type, lifestyle, cosmetic regimens, health/wellbeing, and aftercare of your permanent makeup will contribute to your final results.  

Additional touchups may be needed to achieve your desired results.  In the event that you are unhappy with your results, we ask that you notify Azucena no later than 90 days from your procedure. Together we’ll work to help get you closer to your desired results. Any issues that are brought up beyond 90 days from the last appointment will not be considered and will be billed at the standard touch up rate.

Reviews and Online Behavior

As a small business, word of mouth is of the outmost importance. We do not believe in censorship and welcome honest feedback. However, negative reviews of services without addressing them personally with Azucena prior to publishing will terminate any established working relationship. The option for future services will be forfeited.
